ActivHealthCare’s comprehensive Provider Credentialing Plan includes standards for credentialing and re-credentialing which are compliant with NCQA and URAC guidelines.

Through our credentialing process, we are assuring our networks that ActivHealthCare providers are qualified to offer patients exceptional healthcare. ActivHealthCare is the perfect channel between the plan which needs credentialed professionals to provide employees with the best in healthcare and the busy chiropractic office that needs access to major plans.

As a provider, if you wish to become a member of ActivHealthCare or are already a member provider and wish to re-credential; please chose the appropriate state:

• South Carolina
• Tennessee

Every provider’s credentialing application is reviewed by our staff licensed chiropractor, with final approval given by ActivHealthCare’s Credentialing Committee and Board of Directors.
