Georgia Credentialing/Quality Assurance Program

To become a member of ActivHealthCare, a provider is required to:

• complete a credentialing application appropriate to Georgia;
• submit a variety of documents such as state chiropractic licenses, professional liability certificates/declaration pages, federal tax identification forms, etc.; and
• undergo primary source verification of their credentials.

If you are already a member provider and need to re-credential:

• complete a re-credentialing application appropriate to Georgia; and
• submit current and complete documents such as state chiropractic licenses, professional liability certificates/declaration pages, federal tax identification forms, etc.

Every provider’s credentialing application is reviewed by our staff licensed chiropractor, with final approval given by ActivHealthCare’s Credentialing Committee and Board of Directors.
